Case Manager
Responsible for coordinating services to assure that people have access to a full array of needed community services including appropriate medical; social; educational; and other needed services. The CM is responsible for identifying the person’s needs and resources, coordinating services to meet those needs, and monitoring the provision of all services. Responsible for administering the Home and Community based waivers according to DHHS and DDSN regulations. CM is responsible for annual assessment and plan development, as well as accurate and appropriate billing and documentation. In addition, CM needed to serve as the point of entry for all referrals to the Disabilities Board of Charleston County, provide community resources information to potential families and consumers, and complete intake process.
Candidate will have a good working knowledge of community resources, be extremely organized, be able to manage competing priorities, and will be deadline oriented. They must treat consumers and families with dignity and respect and support their right to choice with helping to protect their safety and well-being. The candidate will represent DBCC and CM in a professional manner at all times regardless of the situation. The candidate must have experience and knowledge of intellectual disabilities and autism spectrum disorders as well as be knowledgeable of the issues facing individuals and families of those who have special needs. Basic computer skills are required.
A bachelor’s degree is required from an accredited college or university, and documentation of at least one year of experience working with the target population. The degree must be from an institution that is accredited by a nationally recognized educational accrediting body.
