Harborview Pharmacy is looking for an outstanding TEMPORARY/HOURLY PHARMACIST 2 (Day Shifts) to join our Inpatient Pharmacy team! The Pharmacist 2 will perform and supervise dispensing functions in the Inpatient, IV, and Outpatient pharmacies.
Pharmacist 2 Dispensing Responsibilities:
General:
- Comply with all state and federal pharmacy laws.
- Supervise pharmacy assistants in all areas of technical responsibilities.
- Fill prescriptions for medication and compound pharmaceutical preparations.
- Ensure that controlled substances are properly stored, dispensed, recorded, and inventoried.
- Supervise the preparation and delivery of any pharmaceutical preparations kept as stock on the nursing units.
- Monitor medication and intravenous solution therapy for rational use.
- Demonstrate ability to utilize available resources to meet patients' age or handicapped-related needs.
- Conduct a thorough patient profile review to assess the appropriateness of the prescribed pharmaceutical including monitoring individual patient profiles for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in patient drug therapy.
Unit-dose Distribution System:
- Interpret and code physicians' orders.
- Ensure proper computer entry of orders.
- Evaluate allergy and drug interaction information.
- Ensure timely delivery of medications to proper patient care areas.
- Ensure that the correct drug and prescribed dose is delivered to the appropriate patient at the proper time.
IV Admixture System:
- Interpret physicians' orders
- Ensure proper computer entry of orders.
- Evaluate allergy and drug interaction information.
- Ensure correct aseptic preparation, labeling and delivery of intravenous solutions.
Outpatient System:
- Interpret physicians' orders.
- Ensure proper computer entry of orders.
- Evaluate allergy and drug interaction information.
- Ensure correct filling of medication containers.
- Ensure proper dispensing of medication to correct patient.
- Provide oral and/or written information to the patient regarding drug therapy.
- Consult with physicians and other healthcare providers regarding all aspects of drug therapy.
Other Responsibilities:
- Provide drug information to patients, physicians, nurses, and allied healthcare providers.
- Direct the work of pharmacy assistants, interns, students, and residents.
- Assist with periodic peer review and evaluation.
- Participate in the implementation of formulary management, optimizing patient care through appropriate selection of treatment options.
- Engage in regular therapy review meetings with other staff pharmacists to discuss drug therapy.
- Participate in Code 199, specifically preparing drugs for administration and recommending agents and doses.
- A valid Washington State license to practice pharmacy or eligibility to obtain license prior to employment.
- One year of experience in pharmacy practice or completion of an American Society of Health Systems Pharmacists accredited residency training program.
Equivalent education/experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/certification/registration.
DESIRED:
Completion of ASHP-accredited residency program or advanced degree preferred.
