UWMC INPATIENT PHARMACY has an outstanding opportunity for a TEMPORARY/HOURLY PHARMACIST 2 (Rotating/Variable Shifts). The Pharmacist 2 will be responsible for performing and supervising the dispensing functions in the Inpatient, IV Admixture, and Satellite Pharmacies.

Responsibilities:

Dispensing:
• Conduct a thorough patient profile review to assess the appropriateness of the prescribed medication including monitoring for drug duplications, interactions, allergies, appropriate dose, strength, and route of administration at the time each order is received; document relevant information in medical record as required.
• Promote rational, cost-effective drug therapy by promoting the use of formulary items and optimizing patient care through appropriate selection of treatment options.
• Direct pharmacy technicians in all areas of technical responsibilities.
• Ensure proper preparation and labeling of all medications.
• Ensure that controlled substances are properly stored, dispensed, recorded, and inventoried.
• Ensure timely delivery of medications to proper patient care areas.
• Ensure that all bulk packaged items are properly prepared, labeled and documented.
• Ensure correct aseptic preparation, labeling and delivery of compounded sterile products.  Perform routine visual observation and oversight to ensure proper garbing, handwashing, cleaning and aseptic technique processes is followed during sterile compounding.

Other:
• Provide drug information to patients, physicians, nurses, and other members of the health care team.
• Participate in protocol development, exploratory projects and committees which contribute to improved patient care.
• Document clinical interventions as appropriate.
• Detect, monitor, document and report any adverse drug reactions and medication errors.
• Assist in developing safe medication practices to prevent errors.
• Participate in teaching pharmacy students, interns and technicians and guiding experiential training.
• Participate in Code Blue response, specifically preparing drugs for administration and recommending agents and doses.
• Comply with all state and federal pharmacy laws. Perform related duties as assigned REQUIREMENTS: State of Washington Pharmacist License or, or eligibility to obtain license prior to employment AND one year of experience in pharmacy practice. Months spent in the completion of an American Society of Health System Pharmacists accredited residency training program may substitute month-for-month for the required experience.

Equivalent education/ experience will substitute for all minimum qualifications except when there are legal requirements, such as a license/ certification/ registration.
